Please download our Patient Registration and Medical History form. You may fill it in on your computer, save it and send it to us. Alternatively, you may print it out, complete it ahead of your appointment and bring it with you.
Our office policy regarding appointments:
We will attempt to contact you to provide a “courtesy” reminder for all appointments 2 days before your appointment. If your appointment was booked several weeks or months prior, then we will also attempt to contact you to remind you 2 weeks before the appointment. This will give you an opportunity to move the appointment if you need to better fit it into your schedule. If we do not hear from you, then we will expect that you will be attending your appointment. Depending on your preference, we can contact you through email, text, or a phone call.
When you book your appointment, we are reserving a specific amount of time to provide your treatment. If your schedule changes unexpectedly, please call our office immediately to inform us of your need to change this reserved time. We require 48 business hours notice if you wish to change or cancel your appointment so that we may have a reasonable opportunity to give the appointment slot to another patient. A fee will be charged, at the current Nova Scotia fee guide rate, for any short cancelled or failed appointment at our discretion. This is a strict office policy as unfortunately, in recent years, there has been an increase in the number of patients who have not shown for appointments or who do not provide sufficient notice to allow another patient to have their time slot,
